Please download the appropriate form (see below), fill out, and bring with you to your appointment.  Contact Christi if you have any questions prior to your scheduled time.

If someone is driving you to your appointment to our home office (Moser Farm Road), we ask that they be able to drop you off for your appointment and return for you after our session has ended.  As it is my intention to offer the highest quality of service, please contact Christi if other arrangements need to be considered.

We ask that you arrive no later than 5 - 10 minutes prior to your appointment time.
